Experts have shared their theories, Trump has responded to the rumors, and social media users continue to offer their own opinions. Now, artificial intelligence (AI) is adding another perspective to the conversation.

Two Doctors Started with Her Earliest Known Photos

Dr. Joel Kopelman and Dr. Gary Linkov began their analysis with photographs of Trump as a teenage model in Slovenia, using those images as a baseline for later comparisons.

“Here we see a photograph of Melania at the age of 17 in her home country of Slovenia when she was a young model, and what we see here is a well-proportioned face,” he said. “Her eyes are oval-shaped, almond-shaped, and have a bit of a flare at the end.”

The second feature he highlighted was her nose. “You can see in this photograph that there’s a little broadening of the dorsum of her nose,” he said. “It is a little broader, and there’s a little fullness of the tip of the nose.”

Linkov’s assessment was similar. He pointed to her full eyebrows, almond-shaped eyes, and balanced lips.

“She also has strong facial bony structure,” he said. “Both her maxilla, or where the cheekbones are, and her mandible, where the chin and the rest of the jawbone is, are both quite prominent.”

1996

By the mid-1990s, Trump had launched her modeling career in New York. Comparing those photographs with her teenage images, Kopelman believed the nose showed signs of refinement.

“There is a suspicion in my mind that she’s undergone some surgery,” he said. “Specifically, it looks like she had a rhinoplasty because the width of her nose has now narrowed.”

He also believed the tip appeared less rounded than before. “It doesn’t look quite as bulbous as it did when she was younger,” Kopelman added.

While he emphasized that photographs alone cannot prove a procedure occurred, the nose became the first feature that led him to suspect surgical intervention.

1998

Although beauty trends of the late 1990s were evident in details such as thinner eyebrows, Linkov believed the more significant changes involved the shape of Trump’s nose.

“I’m also seeing that her nasal tip, to me, looks less bulbous, and the dorsal hump … is less prominent,” he said. “I suspect that she may have had a possible rhinoplasty sometime between the age of 17 and 28,” Linkov said.

1999

As the timeline approached the end of the decade, Linkov turned his attention to Trump’s lips.

Compared with earlier images, he believed they appeared fuller and more pronounced. “I also see signs of potential lip filler that have been placed into her upper and lower lips,” he said.

The observation stands out because the cosmetic landscape looked very different at the time. “The first FDA-approved filler for the face was Restylane in 2003,” Linkov explained.

If any enhancement occurred, he noted that collagen injections and silicone-based products were among the more common options available before modern fillers became widespread.

2000

Both experts were pointing to changes in multiple areas of the face. Kopelman concentrated on the mid-face region, where he believed additional volume had altered the contours of her cheeks.

“It looks like she had some kind of augmentation of some kind,” Kopelman said. “Either a hyaluronic acid gel filler or fat grafting.”

At the same time, Linkov continued examining the nose and identified details he believed supported his earlier rhinoplasty theory. “We see more nostril show on frontal view, which is a very common feature after rhinoplasty,” he said.

He also pointed to changes along the bridge of the nose that he believed could be associated with surgical narrowing techniques. “You can see where osteotomies were likely done,” Linkov said.

2002

By the early 2000s, attention shifted from the nose and lips to the overall shape of the face.

Kopelman focused on the lower cheek area, where he believed a new contour had emerged. “There is some kind of hollowing that’s occurred over what’s called the buccal recess,” he said.

The appearance could have resulted from several factors, including facial structure, expressions, lighting, or cosmetic intervention. Because photographs provide only a limited view, he stopped short of drawing a firm conclusion.

Linkov concentrated on a different detail entirely. Comparing newer images with those from her youth, he noticed changes in the area between the nose and upper lip.

“What I see here is upper tooth show on repose,” Linkov said. “And her philtrum, or the distance between the base of the nose and the tip of the Cupid’s bow, is now longer than it was in her teenage years.”

He noted that the change could be a natural effect of aging, but it can also occur after rhinoplasty. “People who have had a rhinoplasty can sometimes see that the philtrum gets longer as well,” Linkov said.

2004

During this year, one of the most prominent features of Trump’s face was her cheekbones. Linkov believed her cheeks appeared noticeably fuller than in previous years.

“I wonder if perhaps she’s had cheek implants,” he said. “Because there’s so much regularity to both of the cheeks now, and there’s more projection that, I think it would have been hard to get to this state with filler alone.”

He stressed that the observation was speculative and based solely on photographs. Still, it introduced a new theory into the discussion and expanded the list of procedures the experts considered possible.

2005

By 2005, Kopelman believed Trump’s face appeared fuller than it had in earlier years. “But compare this picture to the prior picture, where her face was more V-shaped, a little more hollow temporally,” Kopelman noted.

In one image, expression lines that typically appear during smiling seemed almost absent. “As she smiles, she has no crow’s feet over here. She has no frown lines between her eyebrows,” he said.

A second photograph from the same year appeared to tell a different story. “She has crow’s feet in the corners of her eyes,” Kopelman observed.

2007

By this year, the focus shifted toward one of Trump’s most recognizable features: her eyes. “Melania’s eyes here start to appear smaller, and this has become a more characteristic trait of her face,” Linkov said.

Rather than attributing the look to a single cause, Linkov outlined several possibilities. One theory involved procedures designed to elongate the eyes and create a more lifted appearance.

“But the first possibility is that she may have had a canthoplasty or a lateral brow lift that would have brought out the sides of her eyes,” he said.

According to Linkov, both procedures can contribute to a more almond-shaped appearance by lengthening the eye horizontally. He also suggested that Botox treatments around the forehead and crow’s feet may have started around this time.

2008

The following year brought renewed attention to the same area of the face. Kopelman focused on the contour of Trump’s eyelids, noting that the shape of her eyes appeared especially pronounced in photographs from that period.

“Because it appears that she has this very high sweep to her lower lid, and it’s almost encroaching on the aperture of her eye,” he added.

Linkov continued exploring why her eyes appeared different from earlier years. “At the age of 38, we again see that the eyes appear smaller,” he said.

While Linkov said a canthoplasty or brow lift remained possible, he also suggested another explanation. According to him, repeated Botox treatments can change the position of the brows, making the eyes appear smaller or less open.

2009

As attention moved beyond the eye area, Kopelman pointed to what he considered one of Trump’s most consistent features throughout the years: her youthful-looking skin.

“Melania is retaining her appearance again of flawless skin, almost no wrinkles, and a fullness of her face,” he said.

The surgeon believed facial volume may have contributed to that effect. “Apparently, she’s using some fillers to fill in some areas that are hollow and deep,” Kopelman added.

2012

The next major shift in the timeline came from changes commonly associated with getting older. At age 42, Linkov said he began noticing subtle changes along the lower face. “I’m seeing some early signs of jowling,” he said.

Rather than viewing the development as unusual, he described it as a natural part of facial aging. As tissue gradually descends over time, fullness often begins to gather around the jawline.

“As the cheek complex starts to see some of the effects of gravity, it starts to move down and in, and as it does that, you start to get this jowling that forms right around some of the ligaments,” Linkov explained.

2014

Two years later, Linkov noticed something that appeared to move in the opposite direction.

While previous photographs suggested limited forehead movement, newer images showed greater facial expression and slightly more visible forehead lines.

“At the age of 44, I’m starting to see that there are more forehead wrinkles,” he said.

At the same time, he observed that Trump’s eyes appeared more open than they had in some earlier photographs. “So maybe the dose of her Botox was lowered,” Linkov suggested.

2015–2017

Reviewing photographs from this period, Linkov said he saw no significant changes. “In 2015, at the age of 45, I see no change, and that’s the same through 2017,” he said.

2018

As Trump approached 50, both experts began identifying changes they associated more directly with aging. Kopelman noted that her face continued to appear relatively full, whether due to injectables, genetics, or her natural facial structure.

“She’s starting to show, however, some weakness of the muscles on her neck, and you can see those platysmal bands starting to emerge,” he said.

Around the eyes, Kopelman noticed emerging crow’s feet while remarking that her forehead remained unusually smooth. “But it looks like she doesn’t have a wrinkle on her forehead,” he observed.

Linkov focused on the neck as well and reached a similar conclusion. “We’re starting to see some of that neck skin laxity building and becoming more visible,” he said.

2019

A year later, Linkov identified another subtle change associated with aging. “What I wanted to point out here that the lip corners are starting to get a bit downturned in Trump’s case, and this is very common as we are in our 40s and start to enter our 50s,” he said.

2022

The most significant speculation in Linkov’s analysis came from photographs taken when Trump was 52.

After years of observing increasing laxity around the neck and jawline, he noticed what appeared to be a reversal. “I am seeing that Melania’s jawline looks a bit smoother,” he said.

He also believed the neck appeared tighter than in previous years. “The neck, to me, looks like it doesn’t have as much of the redundancy or banding that I was seeing earlier,” Linkov said. “So perhaps she had a facelift.”

However, he cautioned against drawing firm conclusions from the photos alone. He noted that Trump’s hair often obscured her ears, making it difficult to assess one of the areas surgeons typically examine for signs of a facelift.

He also said that non-surgical skin-tightening treatments can create similar improvements.

2024

Kopelman focused on changes around the lower face and neck. “We see particularly a weakening of the neck muscles, and we can start to see those bands starting to emerge,” he said.

He also pointed to fullness around the mouth area and nasolabial folds, which he believed could be related to fillers. “Her face itself looks like there’s been some filler put in around her nasolabial folds,” Kopelman said.

“We are starting to see some age-related changes that are starting to occur, which, of course, is normal for someone who’s 54,” he said.

How Much Could the Suspected Procedures Cost?

Linkov estimated that the procedures he speculated about throughout his analysis — including a possible rhinoplasty, fillers, Botox, and cheek enhancement treatments — could cost approximately $223,000 if they had all taken place.

While the doctors pointed to changes they believed could be consistent with cosmetic enhancements, Trump has denied undergoing such procedures.

In a 2016 interview with GQ, she said, “I didn’t make any changes. A lot of people say I am using all the procedures for my face. I didn’t do anything.” She also said she does not support injectable treatments.

“I’m against Botox, I’m against injections; I think it’s damaging your face, damaging your nerves,” she said. “It’s all me. I will age gracefully, as my mom does.”

Trump added that she takes care of her skin and body and credits healthy habits for her appearance. However, recent images have continued to fuel speculation about whether she has undergone cosmetic procedures.

On June 14, 2026, she attended UFC Freedom 250 at the White House alongside her husband, President Donald Trump. She wore a fitted black dress, black pointed-toe heels, and a leather jacket draped over her shoulders.

The appearance quickly sparked reactions online.

On Instagram, someone else wrote, “Another nose job.” Another said, “It’s noticeable… too much botox.” On Facebook, one commenter wrote,”She’s looking just like him.”

A different user remarked, “Her eyes are not aligned,” while another asked, “Where are her teeth.” Another chimed in, “And she said she’s never had any work done….okay!”

The debate over Trump’s appearance has also inspired a different type of analysis. Using AI, a series of images explored how she might look if age-related changes had developed without cosmetic intervention.

AI Imagines How Melania Trump Might Have Aged Naturally

While plastic surgeons focused on specific facial features and possible cosmetic procedures, AI offered a different perspective.

Using recent photographs as a starting point, AI-generated images reimagined how Trump might look if her appearance had changed through the natural aging process alone. The images are entirely speculative and do not represent her actual appearance.

Across the generated portraits, deeper lines around the eyes and mouth, more visible skin texture, increased skin laxity, and a softer jawline appeared compared with many recent public photographs.

At the same time, the images preserved features long associated with Trump, including her facial symmetry, eye shape, and overall facial proportions. Some also emphasized age-related changes such as volume loss, wrinkles, and increased laxity around the neck.

These depictions are not medical assessments and should not be interpreted as predictions. Instead, they provide a visual illustration of how AI interprets the aging process when creating alternative versions of a well-known public figure.
